Maddie Bicks is an alcoholic suffering from cirrhosis. Since her alcoholism made her ineligible for a liver transplant, she and her sister Catherine planned for Maddie to use Catherine's ID and be put on the list that way. Unfortunately Maddie started growing impatient and decided to kill the three people above her (Lucy Friedman, Peter Hodges and Veronica Miller) over five days during Catherine's trip to Switzerland by lacing aerosol cans with hemlock. When Catherine found out about this and threatened to go to the police, Maddie went into a rage and accidentally killed her sister by pushing her into a wall in the parking garage of her apartment complex.
When Shawn solves the mystery and the gang go over to her apartment, Maddie starts crying, saying she doesn't know what happened. Gus mentions that Maddie must have suffered from impaired judgment, since one of the side effects of severe liver failure is psychosis. It is therefore safe to assume that Maddie was institutionalized rather than incarcerated.
