Hell Hag is a disheveled old woman who appears in Gus's final dream in S8E9 "A Nightmare on State Street". After he has visited his former therapist Ashford Simpson at a college book signing, the doctor's utterance of "Come get some" causes her to appear out of nowhere. She scares Gus enough to make him wake up and hang out with Shawn in the middle of the night.
The Hell Hag is seen again in Psych 2: Lassie Come Home, but only in a photograph. While Shawn is trying to solve Devon Tileback's mysterious suicide, he says "We've ruled out the Hell Hag" as we see a bloody, crossed-out picture of her on the wall.

- She is portrayed by Psych writer Carlos Jacott, who also appeared as Terrance in S1E5 "9 Lives", Geoffrey in S6E8 "The Tao of Gus", Mortimer Camp in S8E3 "Remake A.K.A. Cloudy... With a Chance of Improvement" and Gus's Boss in Psych: The Movie.
- She is a reference to the Old Woman that appears in Army of Darkness, which Bruce Campbell, who appears as Ash in that film and as Dr. Simpson in "A Nightmare on State Street", says "Come get some" to after she threatens to swallow his soul.
